Creating a Treatment Note and Form Template

Learn how to create, edit, and manage treatment note templates, either by cloning existing forms or creating a new template from scratch.

In Rehab Guru, you can create customised treatment note templates for your clients. You can either clone and modify existing sample forms or start from scratch to create your own. This guide will show you how to create, edit, and manage treatment note templates.

Step 1: Cloning and Editing a Sample Form

  • Navigate to Account>Forms from the left menu bar.

  • Choose a sample form, such as Review Appointment, and click Clone. The cloned template will now appear in your saved list above.

  • To edit the template, click on the Edit button. This will open the template for customization.

  • You will see different sections within the form, such as Subjective Examination. Each section contains prompts or questions that you can modify.

Step 2: Customising Prompts and Sections

  • You can change the Name of any prompt, make it required, or change its Field Type (e.g., paragraph, file uploader, text box, etc.).

  • To reorder prompts, use the arrows or duplicate a prompt if you need to use a similar question multiple times.

  • You can also delete individual prompts or entire sections if they are not needed.

  • If needed, you can duplicate entire sections to create a similar format with different content.

Step 3: Saving and Using the Template

  • Once you are satisfied with the template, click Save. The template will be available in your Saved List.

  • To use the template, go to the Clients page, select a client, and choose New Treatment Note. From the drop-down list, select the form you created.

Step 4: Creating a New Template from Scratch

  • If you prefer to create a new treatment note template from scratch, click Add New Template within the Forms section.

  • Enter a Template Name and a Section Name. Begin adding prompts by clicking Add Question and choosing the appropriate file type for each.

  • Continue to build the template by adding more sections, duplicating sections if needed, and customising the prompts.

  • Once you are happy with the template, click Save, and it will be available in your saved list for use.
