Learn how to create and send forms to clients in Rehab Guru, either manually or through automation, and manage client responses.

Ensure you have created the necessary form templates.

In Rehab Guru, you can create and send forms to your clients either manually or through automated emails. This guide explains both methods and how to manage the responses from your clients.

Method 1: Manually Sending a Form to a Client

  • Navigate to the Clients page and select the client by clicking View.

  • In the client's account, select Forms from the left-hand menu.

  • Click on Create Form on the right side of the screen.

  • Use the drop-down menu to select the form template you wish to send. You can review the form and check for required fields (indicated by a red asterisk).

  • Once satisfied with the form, click Save. You will be prompted to attach the form to an existing Case for the client. Select the relevant case or create a new one if necessary.

  • After saving the form, the Actions button gives you options to:

  • Send the form by Email.

  • Edit the form or case details.

  • Track the status (not sent = orange, sent = yellow, completed = green).

Method 2: Automating the Sending of Forms

  • Navigate to Account>Forms and select the form you wish to automate.

  • Click Copy Static URL to generate a link for the form. You will receive a confirmation that the URL has been copied.

  • Navigate to Account>Customise and scroll down to the email customisation section.

Paste the copied form URL into the email body where appropriate. You can automate form sending for various email types, such as:

  • Exercise programme emails.

  • Programme expiration emails.

  • Appointment confirmation, reminder, or cancellation emails.

  • After customising the email content, click Save. The form will now be sent automatically in the specified emails, and client responses will appear in the Responses section.

Managing Client Responses

  • When a client completes a form, it will show as Submitted (green status). You can preview the form in the Responses section.

  • If the client's email address is already in your system, Rehab Guru will automatically assign the form to their account. If not, you can manually assign it by clicking on the client's name and attaching the form.
