Patient-reported outcome measures (PROMs) in Rehab Guru score themselves. You can use a ready-made PROM from the sample forms, or build your own with a score for every answer and a total that works itself out. This guide covers both, then how to send a PROM to a client and track their recovery.
You need the Notes, Forms & Outcome Measures add-on switched on. See Notes, Forms & Outcome Measures Add-on.
Step 1: Using Existing PROMs
Go to Account, then Content Management, then Forms, and open the Sample Forms tab.
The sample forms are grouped by category. Under Patient Reported Outcome Measures you will find measures such as the Lower Extremity Functional Scale (LEFS), the Shoulder Pain and Disability Index (SPADI), KOOS, HAGOS, FAOS, the Roland-Morris questionnaire, the NPRS and the PSFS.
Click Preview to see every question, then click Clone to copy the form into My Forms, where you can edit and share it.
Sample PROMs already score themselves, so a cloned PROM is ready to send straight away.

Step 2: Creating a New PROM
If the measure you need is not in the sample forms, open the My Forms tab and click Add Template (the blue button at the top right).
Creating a new patient-reported outcome measure template
Enter the name of the outcome measure in place of New Form.
If the measure has different parts, such as pain, function and quality of life, use + Add section to create a section for each one and name it to match.

Step 3: Adding and Configuring Questions
Write each question exactly as it appears in the published measure.
Most PROMs use one answer per question, so choose Single-choice from the question type menu and add each possible answer with + Add option.
Tick Required if the client must answer the question.
Setting up a Single-choice question with its answers
Click Advanced Configuration, tick Outcome Measure to mark the question as part of the score, and enter the Internal Numeric Values: the points each answer is worth. Click Done.
Marking the question as an outcome measure and assigning numerical values
When the next question follows the same format, click the duplicate icon beside the question to copy it, then change the wording.
Duplicating a question for faster template creation
A copied question keeps its answers, but check the Internal Numeric Values in Advanced Configuration on every copy before you save.
Step 4: Adding Auto-Calculation
To have Rehab Guru add up the score, click + Add question and choose Formula from the question type menu.
Adding a formula field for auto-calculation of the PROM score
Every question has its own ID, shown next to Copy ID (for example $afd5). The formula uses these IDs to pick up each answer's value.
The IDs of your questions are listed under the formula box. Click the ones you want to include and Rehab Guru adds them together, for example $afd5+$a728.
You can also edit the formula yourself, for example to turn a total into a percentage: ($afd5+$a728)/4*100.
Using the unique question ID in the formula for auto-calculation
If your formula uses Single-choice or Matrix questions, every option needs an Internal Numeric Value, or the total will not calculate.
Step 5: Saving and Managing the PROM
When you are happy with the PROM, click Save. It appears in My Forms.
To change it later, use the edit icon in the Actions column, or the bin icon to delete it.

Step 6: Sending a PROM and Tracking Recovery
Open the client from the Clients page and choose Forms from the menu on the left.
Click Create form, pick your PROM from the Select template drop-down, then scroll down and click Save. If the client has cases, you will be asked to attach the form to one.
Click the three dots in the Actions column and choose Send by email.
The Status column shows Not sent (orange), Sent (yellow) and Submitted (green).
Creating, sending and tracking a PROM from the client Forms page
To track recovery, send the same measure again at each review. Every submitted form stays on the client's record with its score already worked out, so you can compare their first score with their latest one.
To send a PROM automatically, click Copy static URL under the form's name in My Forms and paste the link into one of your client emails.
